The Yale-New Haven Hospitalist Service
The Hospitalist Service represents a model partnership with Yale-New Haven Hospital, the Yale University School of Medicine Department of Medicine, and private physicians, physician assistants and nurse practitioners in the community.
The Hospitalist Service provides on-site, 24 hour inpatient care at Yale-New Haven Hospital. Each patient admitted to our service is cared for by a daytime attending physician, night-time attending physicians and a physician assistant. Our physicians are Board certified/eligible in Internal Medicine. Our physician assistants are board certified with the National Commission on Certification of Physician Assistants and licensed by the State of Connecticut. In addition, our physicians and physician assistants hold faculty appointments in the Department of Medicine at Yale University School of Medicine. We are committed to the goals of high quality inpatient care, exceptional patient education, and active participation in hospital performance improvement projects.

The Hospitalist Service is an extension of primary care practices from surrounding communities. We have established agreements with many community physicians, physician assistants and nurse practitioners to care for their patients when they are admitted to Yale-New Haven Hospital. Our physicians and physician assistants maintain regular communication about your treatment plan with you and your primary care physician, physician assistant or nurse practitioner and we assist in your transition back to office-based care as it becomes safe and practical.

If you do not have a primary care physician, physician assistant or nurse practitioner in the community and are admitted to Yale-New Haven Hospital, you may be admitted to the Hospitalist Service. One of our attending physicians will be responsible for your care for the duration of your hospital stay. Upon discharge from the hospital, we will refer you to a primary care physician, physician assistant or nurse practitioner in the community for follow-up care.
